Inital Interview

Must pass the written test for skills assessment

Must fill out a criminal background check form

Must present the following upon applying;

Acceptable picture I.D. (any of the following)

US Passport

Resident Alien Card (green card)

Government issued Driver's License or State ID

Social Security Card

Current C.N.A. C.H.H.A. card (if applies)

CPR, Basic Life Support

Result of current physical and TB/PPD Test or Chest X-ray (if skin test is positive)
